WEATHERIZATION SERVICES

Meals on Wheels, Etc. has a Weatherization Program to lower your electric bill. The Weatherization Assistant Program is for low income households.

THIS PROGRAM IS ONLY FOR HOMES THAT HAVE NEVER BEEN WEATHERIZED.

Below are steps to follow in order to be informed or to apply for this program.

STEP 1

Please read about the Weatherization Assistance Program in Seminole-County. After reading, proceed to step 2.

STEP 2

Please read about the Poverty Income Guidelines to determine qualification based on income After reading, proceed to step 3.

STEP 3

Please note all Required Documents for every household members. After reading, proceed to step 4.

WAP Application Information Form
Please fill out your name and date, save in your computer. Remember to email this document back to the Home Program Manager Proceed to step 5.

STEP 5

WAP Preliminary Application Form
Please fill out the form completely and save in your computer. Remember to email this document back to the Home Program Manager Proceed to step 6

STEP 6

WAP Service Utility Bill Agreement Form
Please fill out the form completely, save in your computer. This form must be witness signed in front of Meals on Wheels, Etc. staff or a notary. Please print then proceed to step 7

STEP 7

WAP Authorization for release information
Please fill out the form completely, save in your computer and print. This form must be witness signed in front of Meals on Wheels, Etc. staff or a notary. Please print then proceed to step 8.

STEP 8

WAP Client Intake Form
Please fill out the form completely and save in your computer. Remember to email this document back to the Home Program Manager. Proceed to step 9.

STEP 9

WAP Client Notice Form
Please sign, date and save in your computer. Remember to email this document back to the Home Program Manager. Proceed to step 10.

STEP 10

WAP Client Grievance Policy and Procedures
This form must be witness signed in front of Meals on Wheels, Etc. staff or a notary. Please print then proceed to step 11.

WAP Lead Mold and Moisture Form
